The Archdiocese of Cincinnati requires a parent or guardian to sign a “Permission, Release and Medical Power of Attorney” form for child's participation.

This form covers all Youth Ministry on-going programs and routine activities such as icebreakers, faith sharing, movies, music and prayer. Effective dates are from June 4, 2008 through June 4, 2009. For more information, please contact Youth Minister Maria Meyer at 683-0105.
